About The Position

Prava is looking for a Lead Therapist — PT or OT — to build and lead our therapy presence at Cambridge Village of Wilmington. This is a working clinical leadership role where you will spend meaningful time treating residents while also becoming the person Cambridge Village turns to when it comes to therapy and functional wellness. You will deliver exceptional PT or OT care, build strong relationships with residents, families, caregivers, and community leadership, and establish yourself as a trusted clinical resource. You will identify residents who may benefit from skilled therapy before a decline becomes a crisis, partner with the community's wellness and care teams, and help integrate therapy into the resident experience. You will mentor and support other Prava clinicians as the program grows, maintain strong clinical standards, and use outcomes and functional progress to guide clinical decisions. You will support fall prevention, mobility, balance, strength, cognition, ADL performance, and functional independence, participate in wellness and community programming, and help educate community staff, residents, and families. You will use Prava's technology and AI-enabled documentation platform to reduce administrative burden and help Prava continuously improve how therapy is delivered in senior living. This role requires you to walk the community, meet residents, talk with staff, understand resident needs, and identify opportunities to make a difference. You will have the autonomy to lead your community while having Prava's operating system behind you, combining your clinical expertise with Prava's infrastructure, technology, clinical system, and administrative infrastructure.

Requirements

Current North Carolina licensure as a Physical Therapist or Occupational Therapist
Strong clinical skills
A genuine passion for older adults
Excellent communication and relationship-building skills
The confidence to operate independently
The curiosity to keep learning
The ability to work across Independent Living and higher-acuity senior living needs
A desire to grow into clinical leadership
The ability to balance patient care with program development

Senior living experience is highly valued.
Leadership experience is a plus.

Delivering exceptional PT or OT care to residents within the community
