The Psychiatric Institute of Washington provides comprehensive behavioral healthcare for children, adolescents, adults, and senior adults suffering from mental health and addictive illnesses. Website: www.pschinstitute.com The role:The Lead Patient Care Coordinator (LPCC) plays a crucial role in supporting individuals admitted for high‑risk opioid use by delivering personalized, compassionate care from admission through aftercare planning. Serving as both a guide and advocate, the LPCC facilitates individual and group sessions to help patients identify their needs, overcome challenges, and build effective recovery plans. Leveraging strong knowledge of community‑based resources, they coordinate seamless access to ongoing treatment and support services. As a key member of a multidisciplinary team—including nurses, counselors, therapists, and external partners—the LPCC provides leadership to the peer recovery staff and ensures patients are connected to the continuing‑care resources essential to long‑term recovery.

Requirements

  • Current state or national Peer Recovery Specialist certification
  • BLS certification
  • Three or more years experience as a Peer Recovery Specialist
  • Experience working in a healthcare setting
  • Familiar with the principals of Harm Reduction

Responsibilities

  • Engages individually with patients to build rapport, assess needs and readiness for change, provide education on treatment options, and offer ongoing brief interventions until support needs are met.
  • Communicates key patient information clearly and professionally with the treatment team, following established channels and documentation standards.
  • Facilitates group sessions, providing structured support and completing all required EMR documentation in alignment with hospital policy.
  • Supports post‑discharge continuity of care, coordinating referrals, conducting warm handoffs, and following up with high-risk clients to keep them connected to treatment and recovery resources.
  • Provides administrative and team support, including caseload delegation, new‑staff orientation, and departmental coverage as assigned.
