Serving as both a guide and advocate, the LPCC facilitates individual and group sessions to help patients identify their needs, overcome challenges, and build effective recovery plans. Leveraging strong knowledge of community‑based resources, they coordinate seamless access to ongoing treatment and support services. As a key member of a multidisciplinary team—including nurses, counselors, therapists, and external partners—the LPCC provides leadership to the peer recovery staff and ensures patients are connected to the continuing‑care resources essential to long‑term recovery.
