Lead Options Counselor – social service support coordination
About The Position
Liberty Healthcare is hiring for a full-time Lead Options Counselor position. As a Lead Options Counselor, you will be an active contributor with a statewide Aging and Disability Resource Center system which supports older individuals and people who are living with disabilities. Program objectives include: Functioning as the “go to” resource for people who are seeking information on long term care and other community-based services and supports. Providing person-centered planning in a way that supports informed decision making and the ability to secure services which will enable people to live as independently as possible. This position will require you to travel to communities throughout Kansas for in-person meetings and allow you to partially work from your home office.
Requirements
- Experience leading / managing others
- Outstanding verbal and written communication skills
- Solid computer and organizational skills
- Willingness to use your vehicle to travel throughout the region

Responsibilities
- Leading a team of six field-based Options Counselors
- Ensuring a significant number of Kansans receive high quality options counseling services
- Data analysis and reporting
- Participating in quality assurance initiatives
- Providing guidance and consultation on complex matters
- Meeting with participants and their families in-person and virtually
- Assessing participant’s needs, providing information, and answering questions
- Connecting people with appropriate services and supports
- Documenting interactions, services, and relevant information in computer systems
- Traveling to communities throughout Kansas and participating in onsite meetings
- Creating partnerships with stakeholders
