About The Position

The Lead Medical Navigator works in coordination with interdisciplinary teams to maintain efficient patient flow and quality of care. The role assumes staffing and leadership responsibilities in support of nursing management. All employees in this position are considered ‘Essential Employees’ and may be required to provide mandated shift coverage for programming needs.

Requirements

  • High School graduate or equivalent
  • Diploma/Certificate from an accredited Medical Assistant program, OR an active CT CNA or EMT Certification, OR direct medical patient care experience
  • In lieu of diploma or certificate requirement, at least one year of direct medical patient care may be considered
  • “Direct patient care experience” is defined as medical direct patient care experience, which includes but is not limited to: vital signs, point-of-care testing, phlebotomy, lab processing, facilitating patient flow, etc.
  • Obtain CPR certification within six months of employment
  • AED Training within six months of employment
  • All required credentials to be maintained throughout employment
  • One year of keyboarding and computer skills with working knowledge of Microsoft Office products
  • Demonstrated organizational and multi-tasking skills
  • Good interpersonal/written communication skills
  • Ability to perform duties with minimal direction
  • Customer Service and detail oriented
  • Able to function as a member of a team

Nice To Haves

  • Diploma from an accredited Medical Assistant program or Associate’s Degree in related field of substance abuse and/or mental health is preferred
  • Previous experience in drug & alcohol treatment or disease concept of addiction preferred
  • Previous experience with mental health and/or psychiatric population preferred
  • Familiarity with Alcoholics (or Narcotics) Anonymous 12-Step Program is preferred
  • Certified Nursing Assistant, Emergency Medical Technician-Basic, or Certified Medical Assistant preferred

Responsibilities

  • Process and transition clients through admissions including completion of all required documentation.
  • Meet with client’s family to obtain information related to client’s history as it pertains to admission.
  • Coordinate with all admissions related staff to expedite the clients’ admissions process across Utilization Review, Nursing, Admission Specialists and other related departments.
  • Obtain urine toxicology screens, take client vitals, and report abnormal values/trends to staff nurse.
  • Complete safety checks throughout the clients stay.
  • Orient clients to the rules and expectations for clients at Rushford.
  • Provide basic counseling to help decrease the client anxiety and assure continuity of care.
  • Manage clients’ discharge by ensuring all property is returned, forms are signed, and documentation is complete.
  • Run educational/non-clinical groups.
  • Maintain supplies by managing inventory and ordering as needed.
  • Complete audits as needed.
  • Serve as a liaison between Rushford and outside contracting services, providing prompt service for all requests.
  • Assist in the coordination of programming which may include but is not limited to: staff scheduling, scheduling clinical activities, program development, and staff education.
  • Assumes other leadership responsibilities and initiatives as directed by manager including but not limited to: assisting in the hiring process for new staff, supporting expansion of initiatives, regulatory compliance audits, supporting standard work development, and conducting staff education and training.
