About The Position

The LPN Lead is a key member of the interdisciplinary health care team, working collaboratively with providers, behavioral health consultants, registered nurses (RNs), medical assistants (MAs), care coordinators, and patient service representatives (PSRs) to provide high-quality patient care for an assigned panel of patients. The LPN Lead serves as the clinical liaison for the care team, ensuring clinical staff perform assigned duties effectively and efficiently. This role provides leadership and mentorship to MAs and LPNs and communicates performance concerns or operational issues to the Site Manager. Working within the full scope of LPN licensure, the LPN Lead provides direct nursing care under the supervision of a registered nurse, physician, advanced practice registered nurse (APRN), and/or physician assistant (PA).

Requirements

  • Current Licensed Practical Nurse (LPN) license.
  • Current CPR certification.
  • Demonstrated willingness and ability to lead, mentor, and support Medical Assistants and Licensed Practical Nurses.
  • Ability and willingness to travel on a flexible schedule as needed.
  • Employees who use a personal vehicle for work-related travel must maintain valid automobile liability and property damage insurance.

Responsibilities

  • Obtaining and documenting vital signs and patient health information.
  • Assessing and documenting patients' chief health concerns.
  • Performing, documenting, and following up on laboratory tests.
  • Assisting providers with specialized procedures.
  • Conducting diagnostic tests, including vision screenings, peak flow measurements, pulmonary function testing, electrocardiograms (EKGs), and other clinical assessments.
  • Collecting specimens for laboratory testing through venipuncture, nasal swabs, and other approved methods.
  • Administering medications and injections, including immunizations, and documenting all treatments accurately in the electronic health record.
  • Maintaining required licensure, certifications, and continuing education in accordance with organizational policies and state and federal regulations.
  • Providing LPN coverage at other clinic locations as needed.
  • Supporting a collaborative, patient-centered environment while promoting quality, safety, and compliance.
