Our mental health programs in Solano County provide comprehensive, recovery-oriented services to clients 18 years and older living with moderate to severe mental illness, many of whom also have co-occurring substance use disorders and complex physical health conditions. This position serves both of our Solano County clinics located in Vallejo and Fairfield. Our multidisciplinary team provides psychiatric medical care, case management, therapy, and crisis intervention services in both clinic and community settings, helping clients access housing, primary care, benefits, employment, substance use treatment, and other essential supports. This role is part of a collaborative team dedicated to promoting recovery, stability, and independence through person-centered, trauma-informed care. Community outreach is an integral part of the position, including providing medication support, conducting wellness checks, accompanying clients to medical appointments, and engaging individuals where they live to ensure continuity of care.
