About The Position

This position will be responsible for working with Kinship Caregivers to support placement stability. This position will provide outreach and consultation to kinship families by linking to supports and community resources. This position is responsible for the functions related to GAP and EGAP payments.

  • Minimum of two years experience working with at-risk children and families.
  • Possess a current Background Clearance Screening Letter as required by DCF; and successfully complete the background screening requirements for Community Partnership for Children.
  • If local travel is required, a Valid Florida driver’s license and documentation of current automobile insurance, with $100,000/$300,000 bodily injury liability limit, is required.

Nice To Haves

  • Experience working within the DCF Protective Services system, preferred.
  • Knowledge of the Florida Administrative Code and Florida Statutes, preferred.
  • Bachelors degree from an accredited college or university in human services related field, preferred

Responsibilities

  • Makes contact with kinship families upon placement of children in the home to assess for resource needs and provides information on community resources such as WIC, food pantries, clothing closets, and the Early Learning Coalition.
  • Ensures caregivers are guided to receive eligible funding, completes required documents and communicates with funding entities.
  • Makes referrals for therapeutic and daycare services for children placed in kinship placements.
  • Ensures families are connected with other CPC liaisons such but not limited to: the educational specialist, clinical specialist, psych med specialist.
  • Attend appointments with caregivers as needed.
  • Maintains tracking logs of families and referral sources.
  • Links caregivers to receive CPC’s Kinship Newsletter
  • Refers kinship caregivers with trainings as appropriate.
  • Assist with applying for Care Grants as appropriate.
  • Assist with obtaining funding for extra-curricular or community programs as needed.
  • Enter information into FSFN regarding all contact and referrals.
  • Maintain the kinship closet.
  • Attend shelter hearings and case staffings asssigned
  • Attend community events as appropriate.
  • As the Lead Kinship Navigator, manage all aspects of Guardian Assistance Program (GAP) and Extended Guardian Assistance Program (EGAP) to include ensuring requirements are met for licensed caregivers to be presumptively eligible for GAP, completion of GAP and EGAP Redeterminations in compliance with CFOP, maintaining a tracker, attending staffings
  • Reviews fiscal reports for GAP/EGAP for accuracy
  • Completes GAP/EGAP entry into FSFN
  • Other duties as assigned.
