About The Position

St. Louis Children's Hospital is seeking a Family Care Partner that will be dedicated to ensuring that families receive the support and resources they need during their time at the hospital. This is a crucial role in enhancing patient and family-centered care practices. Our responsibilities include: - Providing direct support to families - Assisting staff in communicating and implementing patient-centered care - Facilitating access to resources within and outside the hospital - Contributing to the Patient Family Centered Care Council - Adapting to the organization's needs regarding patient and family-centered care We are dedicated to ensuring that families receive the support and resources they need during their time at the hospital. The Family Partner provides leadership and support, from the family perspective, in the educational and programmatic activities, practices and policies that promote and integrate patient family centered care throughout the organization. Provides coordinated education for families, staff, and volunteers about core values and behaviors of patient family-centered care. The Family Partner supports effective operations of PFCC programs including the Patient Family Centered Care Council (PFCCC), Family Mentors, Families as Actors in Clinical Simulation, and Family Centered Care Rounds development.

Responsibilities

  • Providing direct support to families
  • Assisting staff in communicating and implementing patient-centered care
  • Facilitating access to resources within and outside the hospital
  • Contributing to the Patient Family Centered Care Council
  • Adapting to the organization's needs regarding patient and family-centered care
  • Act as liaison between requesting departments and program participants by parents/family members of a child who has received care by the hospital.
  • Manage survey/feedback process for participants: follow up with participants as relevant; monitor and update program results on a monthly basis. Track statistics relevant to program and hours of program participants.
  • Manage the new participant intake process: provide information to potential new advisors, assist with paperwork, and conduct training for new participants. Mentor new participants by providing shadowing opportunities and attending events with them until they demonstrate readiness to participate individually.
  • Coordinate requests for program participation on a monthly basis; track events and responses.
  • Manage the request process: field new requests, meet with potential new partners to assess opportunities, gauge roles, and determine how to best meet goals.
