Under the supervision of the Transportation Coordinator, the Lead Driver is responsible for the safe and timely transportation of EveryAge PACE participants. Including transport of participants, meals, durable medical equipment, pharmacy, and any other transportation as deemed necessary. Responsible for overall cleanliness and safety of the vehicle assigned on a daily basis. Responsible for communicating any safety issue to the Transportation Coordinator.

Requirements

  • High school diploma or equivalent required
  • Successful completion of defensive driving course within four (4) months of hire required
  • Cardio Pulmonary Resuscitation (CPR) certification required
  • Have a minimum of one years experience in working with the frail elderly.

Responsibilities

  • Responsible for EveryAge PACE passengers during transport, providing care in a manner which ensures participant satisfaction.
  • Monitors and ensures effective two-way daily communications with Drivers, Transportation Coordinator, and the Inter-Disciplinary Team (IDT), as well as, Center staff via radio or similar communication device.
  • Coordinates and schedules daily delivery of home delivered meals, Durable Medical Equipment (DME), supplies, and needed items from the pharmacy, as well as miscellaneous items as requested by the IDT team.
  • Performs On-Call duties for transportation related call as assigned on a 24 hour basis, as the first on site responder in conjunction with the Transportation Coordinator.
  • Follows assigned schedule, makes adjustments to meet unanticipated changes, while considering the needs of the participant and requirements of the PACE Center. Communicates these changes to dispatch.
  • Operates equipment safely and in compliance with vehicle licensing regulations.
  • Maintains and completes daily required records i.e., Pre- and Post-Trip Inspections, gasoline purchases.
  • Immediately reports, to Transportation Coordinator, any vehicle or cell phone operating problems, malfunction, or incidents out of the ordinary routine.
  • Transports participants to and from medical/specialist appointments, and activity outings, as requested by the Interdisciplinary Team via the plan of care.
  • Transports meals, durable medical equipment, pharmacy, and any other items as deemed necessary for participant.
  • Provides physical assistance to each participant when needed as he/she boards and debarks from the program’s vehicles.
  • Maintains the vehicle in a neat and clean condition.
  • Ensures vehicle equipment is maintained and operating within manufacturers’ recommendations, replacing supplies as needed.
  • Functions as a member of the Interdisciplinary Team when appropriate.
  • Communicates with Transportation Coordinator daily to be updated on participant conditions/ changes, plan of care changes, or Interdisciplinary Team requests.
  • Maintains safe working environment. Follows all EveryAge PACE program policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
  • Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
  • Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.
  • Participates in and supports Quality Improvement Initiatives.
  • Participates in continuing education classes and any required staff and training meetings. Maintains any required certifications or licenses.
  • Coordinate daily routes and driver schedules ensuring safe and timely transport.
  • Mentor and train new hires and provide policy and procedure guidance.
  • Conduct monthly vehicle audits and inspections and ensure fleet is maintained and compliant with safety standards.
  • Maintain accurate records and documentation of fleet maintenance.
  • Demonstrate leadership by example, consistently modeling professionalism, accountability, and a commitment to excellence in all aspects of the role.
  • Foster a collaborative partnership with the co-lead, ensuring alignment on goals, sharing responsibilities equitably, and maintaining open, respectful communication.
  • Work in close coordination with the Transportation Director, providing strategic support, sharing insights, and contributing to the successful execution of transportation initiatives.
  • Performs other duties as required or requested.
