About The Position

The Day Habilitation Lead DSP (LDSP) is responsible for providing direct care and supporting the development, implementation, coordination, and delivery of program services to adults with disabilities in the Day Habilitation Program. This position is expected to implement, supervise, and encourage client participation in social, leisure, and recreational activities while promoting a safe, engaging, and person-centered environment. The Day Habilitation Lead DSP is also responsible for accurately completing and documenting daily goals and service-related activities. In addition, this lead role supports day-to-day operations within the Day Habilitation Program, including client scheduling, activity implementation, documentation auditing, and providing program instruction under the supervision of the Day Habilitation Coordinator. LDSPs will serve as the primary point of contact for DSPs regarding programming and day-to-day needs.

Requirements

  • High School diploma or GED required.
  • Ability to meet physical and mental requirements are necessary to perform all functions of position.
  • Ability to maintain confidentiality.
  • Communication and relational skills to interact in a professional manner with parents, guardians, staff, and clients.
  • Ability to operate a variety of office equipment to include but not limited to personal computers, printers, and scanners.
  • Working knowledge of Microsoft Office Suite software.
  • Complete 9.5 hours of online training within the first 30 days of hire.
  • Must be certified in CPR and First Aid or be willing to obtain certification within 6 months of hire.
  • Certification as a Medication Manager or able to obtain training and certification within 6 months of start date.
  • Complete Safety-Care training within 6 months of start date.
  • Maintain current driver's license and current auto insurance.

Responsibilities

  • Provide accurate and timely written documentation of services in accordance with all applicable funding source regulations.
  • Deliver support to clients in alignment with their Individual Program Plans (IPPs) and individualized needs.
  • Complete all supervisory tasks for direct reports.
  • Maintain current knowledge of updates and changes to HCBS and CARF regulations.
  • Demonstrate strong communication and interpersonal skills when interacting professionally with clients, staff, and parents/guardians.
  • Participate in the planning, preparation, and implementation of daily program activities.
  • Develop and maintain monthly activity calendars that reflect client needs and program goals.
  • Attend and support Community Integration activities as required.
  • Assist with personal care needs respectfully to promote the dignity of clients.
  • Obtain and maintain Medication Manager certification and administer medications as required.
  • Maintain a valid driver’s license and proof of auto insurance in accordance with organizational requirements.
  • Meet the physical and mental demands necessary to safely and effectively perform all essential job functions.
  • Utilize phone systems, computers, and other technology to complete job responsibilities in a timely and accurate manner.
  • Maintain strict confidentiality of all client and organizational information.
  • Follow all safety policies and procedures across program sites and facilities.
  • Provide leadership and guidance as needed using a variety of instructional techniques in both individual and group settings.
  • Conduct internal audits of daily documentation, as assigned, to ensure compliance with regulatory and organizational standards.
  • Complete monthly safety checks and facilitate safety drills in accordance with CARF requirements.
  • Consistently demonstrate and uphold CROSSROADS’ Core Values.
  • Complete all required training within established CROSSROADS timeframes.
  • Actively participate in the Individual Program planning process.
  • Maintain up-to-date working knowledge of changes in the HCBS and CARF rules.
