About The Position

We’re seeking an Advanced Practice Dietitian to provide evidence-based nutrition care, serve as a clinical leader within the healthcare team, and promote positive patient outcomes through innovation and expertise. This role involves covering clinical duties 3 days per week and leadership tasks 2 days per week. The position is located at TouchPoint at Ascension Alexian Brothers in Elk Grove Village, IL.

Requirements

• Registered Dietitian Nutritionist (RDN) with five years of experience.
• Master’s degree in nutrition.
• Licensed Dietitian (or willing to obtain) in the state of practice as applicable.
• Healthcare experience preferred.

• Advanced Practice Certification through the Commission on Dietetic Registration (CDR).

Responsibilities

• Perform comprehensive nutritional assessments and develop individualized care plans to promote recovery and improve health outcomes.
• Provide guidance on nutrition and lifestyle strategies to support sustainable, long-term wellness to patients, families, staff, and the community.
• Apply the latest research in alignment with the diet manual, company policies, and facility protocols to ensure high-quality, consistent care.
• Contribute to and lead QAPI initiatives aimed at advancing patient care and clinical excellence.
• Partner with the food service management team to help achieve patient satisfaction and service goals.
• Support the growth of clinical and departmental staff and dietetic interns through education and training as applicable.
