About The Position

The Lead Diabetic Educator CDE is responsible for performing initial assessments of patients' nutritional, physiological, and metabolic status, providing individual nutrition counseling, and maintaining accurate medical records. This role involves identifying learning needs, demonstrating clinical skills, and managing professional practice. The position also entails scheduling RN/CDE staff for diabetes education classes, coordinating departmental representation and marketing materials for health fairs, and serving as the lead contact for ADA certification, including data collection and submission for yearly Qi projects. Additionally, the Lead Diabetic Educator coordinates with ancillary departments for referrals and scheduling, trains new staff on policies and protocols, and works on monthly reports for patients who decline appointments.

Requirements

  • FL Licensed as a Registered Nurse or Dietitian.
  • Certification from the National Board of Diabetes Educators.
  • Current CPR certification.
  • Current CDE Licensure for Florida.

Responsibilities

  • Perform initial assessment in an ongoing, timely, and systematic manner, focusing on nutritional, physiologic, and metabolic status.
  • Provide individual nutrition counseling to patients and staff per physician’s orders.
  • Maintain medical records of each patient and document information regarding each patient in a timely, accurate, and concise manner according to Clinic and ADA standards.
  • Identify patients’ and families’ learning needs and utilize resources appropriately.
  • Demonstrate competency in certain designated clinical skills such as interpreting lab data, evaluating new products, and interpreting research findings.
  • Demonstrate an awareness of self-responsibility and accountability for own professional practice.
  • Responsible for all scheduling of Rn/CDE staff for coverage of diabetes education classes, adjusting their schedules to allow for evening classes.
  • Coordinates all representation of the department and all marketing materials for health fairs throughout the year.
  • Is the lead contact and data collection/submission for ADA certification.
  • Maintains all monthly data for submission to ADA.
  • Creates yearly Qi project along the guidelines of ADA and handles all data collection and submission.
  • Completes the ADA certification submission application and ongoing data collection.
  • Coordinates with ancillary departments (PFS, Call Center, MCCRS) to allow for appropriate working and scheduling of all referrals.
  • Trains all new staff for the department in policies, protocols, use of cgms, etc.
  • Works monthly reports of referrals for patients that decline to attempt to get them in for shorter education app.
