About The Position

The Lead Community Life Coach plays a dual role, focusing on delivering person-centered care and services within a day program setting while also contributing to the program's overall purpose of helping individuals with developmental disabilities achieve their full potential. This role involves developing activities, coordinating programming, supporting staff development, collaborating on Individual Service Plans (ISPs), and providing daily programming supervision. While not a formal supervisor, the Lead Community Life Coach serves as a primary resource for guidance and support to other Community Life Coaches and can provide backup to the Manager of Day Programs as needed.

Requirements

  • Theoretical or practical understanding of a subject acquired and increases through experience.
  • Proficiencies developed through training, practice, or experience.
  • The level and quality of being able to do something improved with training and experience.
  • Knowledge of serving others, giving & receiving feedback, holding self & others accountable.
  • Skills in collaboration, listening, conflict resolution, and leveraging technology.
  • Ability to achieve quality and continuous improvement of self and processes.
  • High School degree or equivalent preferred, but not required unless directed by state regulations.
  • Previous experience in a similar role and responsibility, work, volunteer, community role, or educational setting that focuses on serving and helping others.
  • Adult CPR and first aid certification.

Nice To Haves

  • 6 months or more previous leadership experience preferred (project management, team/task lead, supervisor, etc.)
  • Drivers’ licensure required if role involves driving others.

Responsibilities

  • Deliver person-centered care and services to individuals, following established plans created in collaboration with the individual, their support network, and external/internal partners.
  • Utilize health and human resource technology for timely and comprehensive documentation.
  • Identify community resources and activities aligned with the interests and abilities of those served, facilitating participation in typical and integrated community activities.
  • Accompany participants to activities as needed, fostering involvement and independence.
  • Communicate progress, successes, and challenges to guardians/care providers.
  • Ensure the personal safety of participants and maintain electronic records of progress, incidents, and challenges.
  • Assist with tours and orientations for new clients and their guardians.
  • Provide personal care and support as necessary, including toileting and medication assistance.
  • Track and document all attendance and transportation, providing data to the Program Manager for billing.
  • Revise pickup/drop-off routes as needed and manage daily communication regarding client call-offs for route adjustments.
  • Welcome, onboard, and train new colleagues in partnership with operational leaders, HR, quality, and nursing.
  • Identify additional training and development needs for all staff.
  • Maintain strong collaborative working partnerships with colleagues, leadership, PWS, and parents/guardians.
  • Ensure consistent implementation of standard operating procedures, including staff schedules, personal funds, ordering supplies, managing receipts, monitoring medical supplies/prescriptions, and conducting quality audits.
  • Serve as the first line of resource for Community Life colleagues, individuals served, and parents/guardians.
  • Bring forward recommendations for quality and process improvements.
  • Provide day-to-day task leadership to Community Life Coaches.
