Under the direct supervision of the Associate Director, Community Partnerships, the Lead Community Health Worker will assist the Associate Director with supervising Community Health Workers, coordinate scheduling, and help deliver comprehensive community health and wellness coordination to patients at various locations, ensuring accessibility to quality care for underserved populations. This role requires a combination of excellent direct patient care skills and team management skills. Responsibilities include assisting with scheduling and supervising Health & Faith Partnership Sites, providing direct patient case management and healthcare access coordination, and leading a collaborative and effective culture for the Health & Faith Partnership team. The Lead Community Health Worker will also ensure that Community Health Workers are up to date on needed trainings to support quality patient support initiatives.
