About The Position

The Lead Clinician provides guidance and supervision to Center for Healthy Families Clinic. She/he assists with program administration including ensuring contractual compliance and meeting program metrics. The Lead Clinician also provides clinical services including one on one counseling and group services for CHF participants.

Requirements

  • Master's Degree, required
  • Minimum 5 years related clinical experience with homelessness, mental health or substance use and with Supervisory experience required.
  • Must be a CT Licensed Clinician (LCSW, LADC, LPC, LPCA, LMFT).
  • Must be computer literate.
  • Demonstrated ability to work in collaboration with other service providers.
  • Must be available to work flexible schedule including evenings, weekends, holidays and other shifts as assigned.
  • Ability to communicate effectively, compile written reports and maintain/review case files.
  • Ability to interact with people of diverse backgrounds required.
  • Ability to communicate effectively.

  • Knowledge with the HIV/AIDS, mental health & substance use disorders (co-occurring) is required.
  • Knowledge of social services in Hartford.
  • Physical and TB Required: Yes, prior to appointment

Nice To Haves

  • Experience in facilitating groups preferred.
  • Knowledge of Human Services, Criminal Justice and the Court systems is preferred.
  • Knowledge of substance use and target population helpful.
  • Counseling background preferred.
  • Bilingual: Yes, preferred

Responsibilities

  • Works along with the Clinical Director to conduct and provide leadership during group activities, and team meetings with CHF clinicians.
  • Assist with onboarding and training needed for new clinicians in process and documentation requirements for all clinical services provided, including EHR System requirements.
  • Provide guidance and assistance with the development and maintenance of the Electronic Health record (EHR) system.
  • Complete CHF reporting needed using data systems and monitoring contractual and departmental requirements.
  • Conduct on-going outreach to community organizations to develop partnerships
  • Represent CRT at community meetings and outreach events.
  • Conduct service coordination with other organizations and resources.
  • Maintains and strengthens relationships with referral sources and document outcomes resulting from such referrals.
  • Complete comprehensive assessments (intakes) for families referred to CHF in the EHR system.
  • Assist with crisis or intervention when needed.
  • Ensures adherence to infection control practices.
  • Assesses customers to formulate treatment needs.
  • Participate in treatment planning and individual and/or group counseling.
  • Makes referrals to outside programs for medical and/or psychological intervention.
  • Attend all trainings as applicable to your credentials.
  • Perform other duties as assigned.
