Requirements

  • Minimum 3 years of caregiving experience in a home care setting
  • Commitment to 40 hours per week
  • CNA certification required
  • Strong communication and interpersonal skills, with a compassionate and patient approach
  • Ability to work independently and manage time effectively
  • Reliable transportation and insurance
  • Ability to pass a pre-employment background check and drug screen

Responsibilities

  • Conduct initial in-home client assessments and develop personalized care plans that support each client's Activities of Daily Living (ADLs) and overall well-being.
  • Provide hands-on personal care, including assistance with bathing, dressing, grooming, mobility, meal preparation, medication reminders, and other ADLs.
  • Serve as a mentor and resource for caregivers by providing training, guidance, and ongoing support to promote quality care and professional growth.
  • Complete routine supervisory visits to evaluate caregiver performance, ensure compliance with care plans, and maintain the highest standards of service.
  • Maintain accurate electronic documentation of client care, observations, and changes in condition while communicating updates to the care team.
  • Collaborate with clients, families, office staff, and healthcare professionals to ensure coordinated, client-centered care.
  • Participate in a rotating weekend on-call schedule to assist with caregiver staffing, scheduling, and client support as needed.
