About The Position

The Lead Care Navigator Medical Assistant/ Non-Medical Assistant will perform a wide range of duties in the medical office. The Lead Care Navigator will help patients navigate the support services in the community and apply for programs needed. The Lead will be a support to in-clinic Care Navigation staff and will meet and network with other Lead Care Navigators within MCC. The Lead Care Navigator MA/Non-MA will have direct contact with patients and work closely with the care team. Applicable to certified medical assistants: As a Medical Assistant, the Lead Care Navigator will assist as a Medical Assistant in examination of patients under the direction of a Physician, Physician Assistant, Family Nurse Practitioner and/or Psychiatrist/Psychiatric Nurse Practitioner. The Lead Care Navigator MA/Non-MA is under the direction of the Care Navigator Supervisors and Care Navigator/ Behavioral Health Operations Manager.

Requirements

  • High School Diploma or equivalent (GED) required.
  • 1-2 years of related work experience.
  • Bilingual Spanish/English is required.
  • Ability to work flexible hours occasionally.
  • Must have strong computer experience with MS Office.
  • Ability to provide excellent customer service to patients and outside customers in person and via phone.
  • Must be able to multi-task duties and carryout projects timely.
  • Fulfill immunization and fit for duty regulatory requirements.
  • Must be able to stand for extended periods of time.
  • Ability to work in fast pace medical environment.

Nice To Haves

  • Certificate from an accredited technical Medical Assisting program preferred but not required.
  • Prior experience working at a community health center a plus.
  • BLS certification is required (Applicable to Certified Medical Assistant only)
  • Epic Electronic Health Records experience a plus.

Responsibilities

  • Primary point-of-contact for in-clinic Care Navigators.
  • Support and train Care Navigators on related MCC clinical and operational processes and workflows.
  • Work with Lead Care Navigators at all clinics in support of the organization-wide Care Navigation Program.
  • Practice strong interpersonal effectiveness skills to maintain good public relations with patients and with colleagues and providers.
  • Maintain patient confidentiality in accordance to department policy and HIPPA
  • Attend and participate in required meetings, training and educational
  • Perform administrative medication/equipment tasks and responsibilities, including checking eligibility, processing BH referrals, verifying demographics, assisting patients with filling out necessary paperwork/forms, and making follow-up
  • Stay current in resources in the community and connect patients to needed resources, such as other community-based organizations, arranging transportation to and from appointments, etc.
  • Request Prior Authorizations and TARs sent by the pharmacy so patients can get ordered medications in a timely
  • Support Behavioral Health patients with scheduling initial and follow -up appointments, initiating BH referrals, assisting with mobile/psych crisis support.
  • Backfill as a Medical Assistant, as needed.
  • Prepare treatment rooms for examination of patients; escort patients to the exam room, interviews patients, measures vital signs and records information on patients’
  • Follow policy and procedures including universal precautions as defined by OSHA’s guidelines on Bloodborne Pathogens and Hazardous Materials
  • Perform other related work as assigned.
