About The Position

Marin Community Clinics, founded in 1972, is a multi-clinic network providing integrated primary care, dental, behavioral, specialty, and referral services. As a Federally Qualified Health Center (FQHC), they serve nearly 40,000 individuals annually in Marin County and have received national awards from HRSA. The Lead Care Navigator (Medical Assistant or Non-Medical Assistant) will perform a wide range of duties in the medical office, helping patients navigate community support services and apply for necessary programs. This role supports in-clinic Care Navigation staff and collaborates with other Lead Care Navigators within Marin Community Clinics. The position involves direct patient contact and close work with the care team. For certified medical assistants, this role includes assisting in patient examinations under the direction of a physician or advanced practice provider. The Lead Care Navigator operates under the direction of the Care Navigator Supervisors and Care Navigator/Behavioral Health Operations Manager.

Requirements

  • High School Diploma or equivalent (GED) required.
  • 1-2 years of related work experience.
  • Bilingual Spanish/English is required.
  • Strong computer experience with MS Office.
  • Ability to provide excellent customer service to patients and outside customers in person and via phone.
  • Must be able to multi-task duties and carry out projects timely.
  • Fulfill immunization and fit for duty regulatory requirements.
  • Must be able to stand for extended periods of time.
  • Ability to work in a fast-paced medical environment.

Nice To Haves

  • Certificate from an accredited technical Medical Assisting program preferred but not required.
  • Prior experience working at a community health center a plus.
  • BLS certification is required (Applicable to Certified Medical Assistant only).
  • Epic Electronic Health Records experience a plus.

Responsibilities

  • Serve as the primary point-of-contact for in-clinic Care Navigators.
  • Support and train Care Navigators on related MCC clinical and operational processes and workflows.
  • Collaborate with Lead Care Navigators at all clinics in support of the organization-wide Care Navigation Program.
  • Practice strong interpersonal effectiveness skills to maintain good public relations with patients, colleagues, and providers.
  • Maintain patient confidentiality in accordance with department policy and HIPPA.
  • Attend and participate in required meetings, training, and educational sessions.
  • Perform administrative medication/equipment tasks, including checking eligibility, processing BH referrals, verifying demographics, assisting patients with paperwork/forms, and making follow-up calls.
  • Stay current on community resources and connect patients to needed services, such as other community-based organizations and transportation arrangements.
  • Request Prior Authorizations and TARs from the pharmacy for timely patient medication fulfillment.
  • Support Behavioral Health patients with scheduling initial and follow-up appointments, initiating BH referrals, and assisting with mobile/psych crisis support.
  • Backfill as a Medical Assistant, as needed.
  • Prepare treatment rooms for examinations, escort patients, interview patients, measure vital signs, and record information.
  • Follow policy and procedures, including universal precautions as defined by OSHA’s guidelines on Bloodborne Pathogens and Hazardous Materials.
  • Perform other related work as assigned.
