Marin Community Clinics, founded in 1972, is a multi-clinic network providing integrated primary care, dental, behavioral, specialty, and referral services. As a Federally Qualified Health Center (FQHC), they serve nearly 40,000 individuals annually in Marin County and have received national awards from HRSA. Their mission is to promote health and wellness through excellent, compassionate care for all. The Lead Care Navigator (Medical Assistant or Non-Medical Assistant) will perform a wide range of duties in the medical office, helping patients navigate community support services and apply for necessary programs. This role supports in-clinic Care Navigation staff and collaborates with other Lead Care Navigators within Marin Community Clinics. The position involves direct patient contact and close work with the care team. For certified medical assistants, this role includes assisting in patient examinations under the direction of a physician or advanced practice provider. The Lead Care Navigator operates under the direction of the Care Navigator Supervisors and Care Navigator/Behavioral Health Operations Manager.
