The Lead Care Manager under the direction of the Cal AIM Services Director, assists and aids members with enhanced coordination of care. The Lead Care Manager is responsible for assessing members to identify needs such as personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments to determine the nature and degree of assistance required. They develop and monitor the member’s care plan, revising it as needed, and coordinate with external entities providing member care, including primary care providers, specialists, behavioral health, community-based services, long-term services and supports, and oral health providers. Responsibilities also include medication review/reconciliation, scheduling and reminding members of appointments, coordinating transportation, identifying and overcoming barriers to care plan adherence, and promoting/coordinating services to meet identified needs. The role involves assisting members, families, and caregivers with plans for meeting needs through culturally appropriate, person-centered planning, education, training, and care instructions, documenting all activities and follow-up plans. Additionally, the Lead Care Manager advocates for and refers members to community resources, compiles records, prepares reports, submits claims to Managed Care Plans (MCPs) when required, and maintains client data in the mylifesteps.org database. They also perform administrative assignments, review service plans, determine the quantity and quality of services provided, access and record member and community resource information, and secure supplementary information as needed. The position also provides comprehensive housing transitions and housing tenancy navigation support, completing screenings and assessments to identify housing needs and factors contributing to instability, facilitating access to suitable housing options, application processes, move-in activities, rental negotiations, and service referrals. This includes conducting facility and habitability inspections, fostering working relationships with property management firms, owners, housing services agencies, and advocacy organizations, and engaging with landlords to prevent and address tenant issues. The role also involves providing culturally appropriate health education information and outreach in community-based settings for various MCP members.
