About The Position

EA Family Services is hiring a full-time Lead Care Manager for the Bakersfield area. This position involves working with a dynamic team to provide care coordination/case management for Medi-Cal enrollees. The ideal candidate will have a passion for working with people, a respectful and objective attitude, and be friendly and outgoing. A Bachelor's degree in a social services field and bilingual English/Spanish proficiency are required. The hourly pay is $23.00. Candidates must also pass DOJ/FBI/CAIC clearances.

Requirements

  • BA/BS in the social services field.
  • Bilingual, speaking both English and Spanish.
  • Must pass DOJ/FBI/CAIC clearances.
  • CPR Certification (Required)

Nice To Haves

  • Experience in case management: 1 year (Preferred)
  • Willingness to travel: 25% (Preferred)

Responsibilities

  • Coordinating with individuals and/or entities to ensure a seamless experience for the member and non-duplication of services.
  • Engage eligible members.
  • Oversee provision of ECM and CS services, including the completion of the assessment, development of a care plan, and implementation of the care plan.
  • Complete referrals on behalf of Members.
  • Offer services where the member lives, seeks care, or finds most easily accessible and within managed care plan (MCP) guidelines.
  • Connect the member to other social services and supports they may need, including transportation and Community Supports.
  • Advocate on behalf of members with healthcare professionals.
  • Use motivational interviewing, trauma-informed care, and harm-reduction approaches.
  • Coordinate with hospital staff on discharge plan.
  • Accompany member to office visits, as needed and according to MCP guidelines.
  • Monitor treatment adherence (including medication).
  • Provide health promotion and self-management training.
