Requirements
- Current state Registered Nurse or LPN license as required by location.
- Registered Nurse experience—Two years of experience with adult medicine and pediatric patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital medical-surgical setting, within the past three years.
- LPN experience—a minimum of five years’ primary care practice experience with adult medicine and pediatric patients in a primary care/family practice setting.

- Care management experience preferred.
Responsibilities
- Lead and mentor care coordinators, providing guidance, training, and support to ensure consistent execution of care coordination strategies.
- Serve as the primary liaison between interdisciplinary teams, practice managers, and leadership to drive organizational goals related to quality, care coordination, and financial performance.
- Develop and manage in-person and virtual meetings with providers and staff to communicate results, share best practices, and identify improvement opportunities.
- Monitor compliance with payor and regulatory requirements and proactively address gaps.
- Prepare and distribute meeting documentation and performance reports.
- Act as a patient advocate and champion for quality improvement initiatives.
- Perform additional leadership duties as assigned by the supervisor.
