Job Details Job Title: Lead Care Coordinator Axis Senior Resource dba Axis Case Management is hiring an experienced Lead Care Coordinator to provide support to our members. ASR is a highly rated Enhanced Case Management Company contracted with the California Cal Aim Program. Servicing over 500 members within 5 Health Plans. Job Type: Full Time- Hourly Job Location: This is a hybrid role. Time will be divided between the office in San Jose, employee’s home office, and traveling throughout Santa Clara County visiting clients, Clients include elderly and individuals experiencing homelessness. Job Schedule: Monday–Friday (40 hours/week) - No overtime and no weekends

Requirements

• Strong time management and organizational skills
• Accurate and timely documentation skills
• Compassionate, professional, dependable, and self-motivated
• Comfortable working independently in the field
• No degree required
• 6 months to 1 year of experience in case management, social work, or community-based services preferred
• Experience working in the community and with vulnerable or diverse populations preferred
• Valid Driver’s License
• Active Car Insurance Policy
• Clear TB test
• Authorization to work in the US
• Ability to travel throughout Santa Clara County using a personal vehicle (mileage reimbursed)

Responsibilities

• Manage an assigned caseload of approximately 30 members
• Create and manage your own schedule while meeting program requirements
• Conduct monthly in-home visits with members
• Provide care coordination and advocacy for seniors, individuals experiencing homelessness, and individuals with substance use challenges
• Connect members to medical, behavioral health, and community-based resources
• Maintain accurate, timely, and compliant documentation
• Collaborate with health plans, providers, and community partners to ensure continuity of care
