About The Position

This role provides regional (site-specific) non-patient care leadership of APPs and serves as a resource to APPs in addressing employee relations issues. The Lead APP VBC also acts as a resource in the development of HOPCo Value-Based Care relationships, collaborates with other Lead APPs and the VP of Value-Based Care for APP recruitment and onboarding, and provides analysis of quality metrics. Responsibilities include conducting performance evaluations, assisting in creating work schedules, and providing APP training and education. Additionally, the role involves assessing patient health status, providing acute and chronic patient care, triaging patient calls and emergencies, documenting patient information, educating patients and families, consulting with staff, and maintaining patient confidentiality. The job holder must demonstrate current competencies for the job position and perform other assigned duties.

Requirements

  • Current state APP license, certified by National Commission on Certification of Physician Assistants or Accreditation Board of Specialty Nursing Certification (ABSNC), current CPR certification.
  • At least one year of experience in an APP leadership role.
  • Knowledge of acute and chronic protocols, administration of medications, delivering physical care following the treatment plan.
  • Knowledge of how to assess patient health and measure/record physiological and growth indices.
  • Knowledge of health promotion principles and techniques and patient learning measurements.
  • Knowledge of quality metrics and standards.
  • Skill in gathering and analyzing physiological, socioeconomic, and emotional patient data.
  • Skill in accurately evaluating patient problems in person or via phone and providing appropriate advice, intervention, or referral.
  • Skill in evaluating and revising care plans based on patient changes.
  • Ability to make responsible decisions within the scope of the dependent medical practitioner.
  • Ability to develop health education programs and materials.
  • Ability to work collaboratively with all members of the health care team.
  • Ability to analyze and respond to data.
  • HOPCo Mission, Vision, and Values must be read and signed.

  • MS preferred.
  • Experience in orthopedics is required.

Responsibilities

  • Triages patient calls and evaluates patient problems.
  • Triages emergencies.
  • Documents patient information and care provided in patient records and maintains patient statistics.
  • Educates the patients and families as appropriate.
  • Provides continuity of care.
  • Consults with other staff as necessary.
  • Maintains patient confidentiality.
