About The Position

Spectrum Healthcare Resources has an opportunity for a civilian LCSW - Special Needs Coordinator (SNC) at Minot Air Force Base. This Social Worker - Special Needs Coordinator contract position offers an outpatient clinic setting with Monday through Friday hours, no nights, no weekends, and no on-call responsibilities. The role includes 11 Paid Holidays and full benefits are available, including Health, Dental, Vision, 401(k), and more.

Requirements

• A minimum of two years full-time experience in care coordination, discharge planning or case management in a medical setting within the last four years.
• Must have a Master of Social Work (M.S.W.) degree from a graduate school of social work fully accredited by the Council on Social Work Education (CSWE).
• LCSW License from any state.

Responsibilities

• Sufficient knowledge to effectively apply Special Needs Coordination functions (Assessment, Planning, Facilitation & Advocacy)
