About The Position

ACLD is a leading not-for-profit agency that assists children, teens, and adults with developmental disabilities in reaching the highest level of independence. We are seeking motivated individuals to assist adults diagnosed with intellectual/developmental disabilities in becoming self-sufficient and self-actualized. This is an entry-level career opportunity in the healthcare/education field. The Job Coach provides on-site job coaching and advocacy for individuals in the intensive and follow-along phases of Supported Employment.

Requirements

• High School Diploma or equivalent
• Minimum of one year experience providing vocational or pre-vocational services to supported persons preferred
• Complete (within first year of employment) twenty-four (24) hours of OPWDD’s Innovations in Employment Supports training
• Clean, valid New York State driver’s license

Nice To Haves

• Associate’s Degree in Psychology, Sociology, Education or related field preferred

Responsibilities

• Provide on-site job coaching for designated caseload
• Complete and ensure accurate and timely documentation of SAP plans, case notes, monthlies, DDPs and other required documentation
• Conduct observational assessments on site to assist the person supported in learning the tasks of the job
• Travel train on the public transportation system and/or roadways
• Maintain case files for each person supported
• Prepare all documentation for ACCES and OPWDD on a monthly basis
• Act as a liaison between the person supported, family and employer
• Attend all staff meetings, Life Plans, conferences and annual training requirements
• Provide overall assistance as assigned
