About The Position

We are looking for an IT Support & Systems Administrator to support our remote workforce and help keep Form Health’s internal systems running smoothly as we grow. This person will serve as a frontline IT resource for employees, own and triage Form’s IT Help Desk queue, and contribute to systems and process improvements that strengthen security, compliance, and the overall employee experience. This is a full-time role reporting to our Head of IT and working closely with our Senior Corporate Systems Engineer and teams across Form Health.

Requirements

  • 3+ years of experience in fast-paced startup and/or healthcare environments and are comfortable adapting as priorities evolve
  • You enjoy being a trusted resource for employees and are comfortable balancing frontline support with project-based work that improves systems and processes over time
  • You are able to support both technical and non-technical users with patience, empathy, and strong communication
  • You are organized, responsive, and dependable, with the ability to manage competing priorities while maintaining a high level of follow-through
  • You bring a thoughtful, service-oriented approach to IT support and genuinely enjoy finding ways to make systems, workflows, and the employee experience better

Responsibilities

  • Serve as the front line of IT support for the Form Health team, helping our remote workforce stay productive, connected, and secure. You will support employees across the tools they rely on every day, including laptops, SaaS applications, collaboration platforms, and other core business systems.
  • Own and triage Form’s IT Help Desk queue, serving as the first point of contact for incoming issues, requests, and support needs. You will resolve tickets directly, know when to escalate, and work closely with the right stakeholders to ensure issues are handled in a timely, thoughtful, and effective way.
  • Provide day-to-day support across employee technology, including endpoints, access requests, account troubleshooting, and business applications. In our remote environment, this role plays an important part in helping employees stay productive and supported.
  • Support onboarding and offboarding through a combination of automated and semi-automated workflows, ensuring new hires have a smooth start from day one and that access is managed securely and efficiently throughout the employee lifecycle.
  • Contribute to broader Corporate Systems and IT initiatives that strengthen Form Health’s security, compliance, and overall employee experience. This may include workflow automation, documentation, process improvements, identity and access management support, and other operational projects that help the company scale effectively.
  • Identify opportunities to improve how IT support is delivered over time by recognizing recurring issues, refining documentation, reducing friction, and helping build a more proactive and efficient support function.
