About The Position

Sunnyhill, Inc. is seeking a Personal Outcomes Coach for their Independent Supported Living Assistance (ISLA) Program. This role involves providing and overseeing training in daily living skills to individuals with intellectual and developmental disabilities (IDD) to help them achieve their personal outcomes. The position emphasizes creating a joyful, kind, and fun environment, ensuring meaningful moments and positive interactions. The coach will support participants in achieving goals outlined in their Individualized Support Plans (ISPs), maintain detailed progress notes, attend meetings, and communicate with individuals, guardians, and service providers. The role also includes assisting with safety plans and maintaining safety supplies, all while representing Sunnyhill with professionalism and a positive attitude.

Requirements

  • A passion for helping others and a commitment to empowering individuals with IDD.
  • Strong organizational skills and attention to detail.
  • Excellent communication skills, both written and verbal.
  • A positive, can-do attitude and the ability to work both independently and as part of a team.
  • Previous experience in a similar role is a plus, but we’re also excited to train motivated individuals who are new to the field!

Responsibilities

  • Provide continuous support and training to help people achieve their personal goals as outlined in their Individualized Support Plans (ISPs), all while bringing a sense of kindness and encouragement.
  • Ensure that all guidelines are met or exceeded and do it with a joyful spirit that makes the process enjoyable for everyone involved.
  • Keep detailed progress notes daily, ensuring compliance with funding guidelines while celebrating every win, big or small, and making sure to have fun along the way.
  • Attend meetings, maintain excellent documentation, and communicate consistently with individuals, guardians, and service providers—all with a smile, a positive attitude, and a sense of fun.
  • Assist with developing and reviewing individualized safety plans and maintaining safety supplies, always prioritizing the well-being and comfort of the individuals while keeping the environment lively and engaging.

Benefits

  • Sign-on bonus of $750 - $1000 (dependent on location)
