The ISCH Worker accepts responsibility for their own actions and empowers individuals to make informed contributions to decisions about their education, health, and care provision to enable them to achieve better outcomes. This role involves assisting in the development of information concerning care plans, working with and encouraging partnership between individuals and the community, and collaborating closely with the Interdisciplinary team involved in an individual's care. The ISCH Worker provides resources to assist the individual in maintaining safety and independence in the community, assists with training and supervision of caregivers and PCAs, undertakes home visits, and documents activity. They also carry out clerical and administrative duties, provide information on their own health status and experience, accept only assignments for which they are qualified, and attend agency orientation, in-service education programs, and staff meetings as required. The role requires practicing safety and competency within the job description, communicating with the agency about problems and concerns, complying with State/Federal regulatory acts and agency policies, and communicating with office staff regarding all scheduling of services to clients. Completing required documentation accurately and on a timely basis, respecting and maintaining client confidentiality, and reporting concerns of client abuse or neglect are crucial. The ISCH Worker refrains from discussing personal problems with clients, establishes and maintains a therapeutic relationship, maintains access to an email account for client updates and schedule changes, and maintains a clean, insured vehicle for client transportation. They work within the aims and objectives of the individual service plan and the policies and procedures of the service, undertake further training as required, and work flexibly to meet occasional requirements outside normal hours. The role also involves utilizing effective time management, attending supervision and meetings, and undertaking other duties commensurate with the post.
