About The Position

Wisconsin Lutheran Social Services of Wisconsin & Upper Michigan (LSS) is seeking a compassionate, organized, and self-motivated IRIS Consultant to join our IRIS (Include, Respect, I Self-Direct) team. In this hybrid role, you will support older adults and individuals with disabilities who self-direct their long-term care services, helping them maintain independence, achieve personal goals, and live meaningful lives within their communities. As an IRIS Consultant, you will build trusting relationships with participants, assist with developing and monitoring individualized support plans, connect people to community resources, and ensure health and safety needs are met. This position combines remote work, community visits, and collaboration with team members across northern Wisconsin.

Requirements

  • Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Nursing, Special Education, or a related field, plus at least one year of supervised experience supporting older adults or individuals with disabilities; OR Four years of direct experience providing social services to older adults and/or individuals with disabilities in community-based settings; OR Current IRIS Consultant in good standing with an approved Wisconsin DHS exemption.

Responsibilities

  • Partner with participants to develop and maintain person-centered IRIS Support and Service Plans.
  • Support individuals in understanding the IRIS program and maintaining eligibility.
  • Conduct monthly participant contacts and quarterly in-person visits.
  • Connect participants with community resources, providers, and support networks.
  • Build relationships with local agencies, service providers, and ADRCs.
  • Complete required documentation accurately and within established timelines.
  • Maintain compliance with Wisconsin IRIS program requirements and quality standards.
  • Work collaboratively with a supportive team while managing an independent caseload.
