Requirements

High School Diploma or GED and extensive experience in incident management, investigations, behavioral health, or human services
Valid driver's license with a clean driving record
Ability to travel to assigned program locations
Ability to pass all required background screenings
Strong written and verbal communication skills
Proficiency with Microsoft Office and electronic documentation systems

Nice To Haves

Bachelor's degree in Human Services, Criminal Justice, Social Work, Psychology, or a related field
Certified Investigator credential
Minimum three years of investigative experience within Behavioral Health, Intellectual and Developmental Disabilities, or Human Services settings
Experience with state incident management systems and regulatory reporting requirements
Knowledge of incident management, compliance, quality assurance, and risk management practices

Responsibilities

Receive, review, and manage reportable incidents across assigned programs.
Conduct thorough investigations in accordance with organizational policies, state regulations, and federal guidelines.
Interview staff, individuals served, families, witnesses, and other stakeholders using trauma-informed practices.
Prepare detailed, objective, and timely investigative reports and supporting documentation.
Collaborate with program leadership, Human Resources, and other departments regarding investigation outcomes and corrective actions.
Monitor incident trends, identify root causes, and recommend quality improvement initiatives.
Participate in incident review committees and follow-up planning activities.
Assist in developing and monitoring corrective action plans to ensure successful implementation.
Serve as a subject matter expert on incident reporting systems, documentation standards, and regulatory requirements.
Provide training and support to staff on incident management processes and best practices.
Maintain professional relationships with regulatory agencies and external stakeholders.
Travel to program locations as needed to conduct on-site investigations and reviews.
Maintain strict confidentiality and uphold the highest ethical standards in all interactions.
