About The Position

The goals of an Interventionist are to keep families together while continually assessing for risk and safety issues. Interventionist provides a family-focused approach, with weekly visits, that is designed to motivate and link families to resources and services to address referral behaviors and family risk factors. Interventionists help the families to generalize gains and new skills by locking in these changes and planning for the future.

Requirements

  • Bachelor’s degree in Social Work, Mental Health Counseling, Psychology or in related field
  • Good writing and oral communication skills; computer literate
  • Excellent time management skills with the ability to meet deadlines in a timely manner while managing competing priorities.
  • Excellent written and oral communication skills.
  • Proficient use of Microsoft applications; knowledge of PROMIS/CONNX desired.
  • Cultural awareness and sensitivity when serving clients that come from different backgrounds and walks of life.
  • Authorized to work in the U.S.
  • Ability to work in-person in New York City (NYC)
  • Ability to travel to other Rising Ground Sites (if required)
  • Ability to travel within the community and do visits (Work-related travel expenses will be reimbursed by agency)

Responsibilities

  • Conduct assessment including review of referral information, identifying and engaging key participants, identifying systemic risks and protective factors, and developing an analysis of the problem behaviors within the ecological context and model process.
  • Engage primary caregiver and other key participants in active change-oriented treatment by identifying and overcoming barriers to engagement.
  • Implement a problem conceptualization, identify community-based resources to provide family specific intervention, monitor community-based intervention and outcome review and strategy revision procedure using the model's process.
  • Maintain clear and concise documentation of treatment efforts that promote peer and supervisory review and feedback, and that demonstrate compliance with model Analytic Process and NYS Connection System requirements.
  • Collaborate with all relevant systems and key participants within each system to ensure their buy-in and cooperation throughout treatment.
  • Participate in all model program training, supervision and consultation activities.
  • Provide home-based interventions weekly.
  • Other related duties as assigned by supervisor.
