About The Position

The Home-Based Crisis Intervention (HBCI) Interventionist provides intensive, short-term, in-home crisis services to children and adolescents ages 5–21 who are experiencing acute psychiatric crises that place them at risk for psychiatric hospitalization. Under the supervision of a licensed clinical supervisor, the Interventionist works collaboratively with families to stabilize the crisis, assess needs, and build coping strategies to prevent future hospitalizations. The Interventionist provides direct support services, assists with treatment planning, and implements behavioral strategies and interventions to promote emotional regulation, problem-solving, and safety within the family environment. The Interventionist also coordinates care with schools, community providers, and other support systems to ensure continuity of care. Services are typically delivered multiple times per week, with flexible scheduling (including evenings and weekends) to meet family needs.

Requirements

  • Bachelors in human services, psychology, social work or other related field required
  • 1-2 years experience in case management, community based mental health, and other related responsibilities required
  • Driver's License and ability to maintain insurability throughout employment required

Nice To Haves

  • Masters in Social Work, Psychology, Social-Welfare, Human Services or other related fields preferred
  • Licensed mental health provider or unlicensed Masters preferred

Responsibilities

  • Provide supportive clinical services to 2-3 families at a time, conducting regular to daily meetings with each child/youth and family.
  • Deliver crisis intervention, prevention, and resolution strategies.
  • Engage in frequent outreach to clients and their families.
  • Conduct strength-based assessments to understand clients' social determinants of health, mental well-being, resilience to trauma, potential risks, and substance use factors.
  • Create personalized service plans tailored to individual family needs.
  • Design and implement effective service delivery strategies.
  • Collaborate with partner service providers to coordinate comprehensive treatment plans.
  • Maintain thorough case management records to ensure effective service delivery.
