About The Position

The Intervention Specialist is a 12-month position primarily responsible for responding to hospitals, communities, and other appropriate locations as a first responder to potential program participants who present to the hospital following a violent injury. The Intervention Specialist will engage potential program participants to forge important relationships with program participants, friends, family, and the community in an attempt to assess the potential for retaliatory behaviors and to encourage engagement in psycho-social resources and other services. The Intervention Specialist will have direct knowledge and understanding of the target geographic area and/or strong relationships and ties to the community.

Requirements

  • Entry level knowledge, lived experience or basic employment skills
  • High School Diploma
  • Valid NJ driver's license
  • Computer, copier, facsimile, and shredder.
  • Driving record to pass check and ability to drive in day/night and reasonable weather conditions.
  • Annual, CPI , AHA BLS and HeartSaver First Aid required within 90 days of hire or transfer

Responsibilities

  • Responds on site to the hospital, community, or home of potential participants immediately or as soon as possible following a violent community injury.
  • Completes program consents and gathers pertinent data and information from the participant via screening and assessment tools and enters it into the electronic health record within 24 hours.
  • Manages and carries a caseload of up to 20 program participants with varying degrees of weekly contact responsibilities.
  • Provide daily, weekly, or monthly contact to participants based on the assessed level of support needed and accurately document case management services.
  • Provide strength-based, trauma-informed, motivational support to participants to ensure that they graduate from the program.
  • Coordinates with the Violence Intervention Team to provide linkages to critical psycho-social services as determined by assessment.
  • Accompany program participants and/or family to community-based services, when indicated.
  • Develop, implement, monitor, and update the Individualized Violence Prevention Plan (IVPP) (Service/Treatment Plan) which is used to mitigate or resolve vulnerabilities to community violence.
  • Professionally engaging participants, family, friends, and community partners providing critical services to participants and/or families and friends.
  • Documents all aspects of interactions with program participants according to set standards within the Electronic Health Record within 24 hours of occurrence.
  • Engages in all trainings and learning activities with enthusiasm.
  • Provides relevant feedback to the Program Director, Senior Therapist, Lead Intervention Specialist, or the Violence Intervention Team.
