This role is a full time 40-hour overnight position with 3 12-hour shifts and 1 4-hour pm shift. The Intervention Specialist provides immediate trauma-informed emotional support, advocacy, and resources to hospitalized patients following trauma. This includes connecting patients with the clinical team, assisting with discharge planning, leading support and psychoeducation groups, and safety planning. The specialist also completes assessments and facilitates warm-handoffs to outpatient providers for ongoing care. Additionally, the role involves responding to trauma alerts, safety calls, pre-escalation needs, collaborating with street outreach teams, and facilitating mediation to mitigate retaliation risks. Patient support is provided through follow-up calls, connecting patients to resources, and sending letters to discharged patients to connect them to outpatient resources. The specialist schedules patient appointments with the TRC, other Advocate internal resources, or community partners before discharge to ensure ongoing care, which requires strong collaboration with other providers. They also participate in the facilitation of psychoeducational and support groups, conduct intakes and needs assessments, and link patients to appropriate resources and outpatient services. This may also require notifying other hospital providers of unmet needs to be addressed while in the hospital. Accurate records of patient interactions and action plans are maintained. Collaboration with the hospital treatment team and community partners ensures comprehensive treatment, discharge planning, and continuity of care.
