About The Position

Internal Medicine Physician Patients do not need another unsigned note. They need a physician who closes the loop—diagnosis, treatment, and follow-through that keeps chronic disease controlled and avoidable hospital stays down. You will own a panel, lead a care team, and turn complex internal medicine into plans people can actually live with. This role matters because continuity, not volume, is what changes outcomes.

Requirements

  • MD or DO; board-certified or board-eligible in Internal Medicine; unrestricted license and DEA.
  • Recent outpatient practice managing adult multimorbidity, or residency continuity experience ready for panel ownership.
  • Documented results on quality measures, transitions of care, and timely notes—not just years on a CV.
  • Skill leading a care team and explaining plans to patients with several conditions and limited health literacy.
  • Eligibility for privileges if the model includes inpatient work.

Responsibilities

  • Own an adult primary-care panel so diabetes, hypertension, heart failure, and COPD stay controlled and avoidable admissions fall.
  • Finish each visit with a documented plan, medication reconciliation, and a clear next step before the patient leaves.
  • Close preventive, immunization, and quality gaps on your panel every quarter against published group targets.
  • Own transitions: every discharge you are responsible for has a timely visit, reconciled meds, and a named owner.
  • Direct APP, nursing, and care-management partners so routine work sits at the right level and your time goes to decisions that change care.
  • Hold access to target—third-next-available and same-day capacity—without cutting visit quality or after-hours chart burden.
  • Improve at least one protocol or peer-review standard each year so the practice gets sharper, not just busier.
