About The Position

Ensure the implementation of the WV Birth to Three (WVBTT) Individualized Family Service Plan process. This role involves contacting assigned referrals within established timelines, informing families of the WVBTT Procedural Safeguards, and establishing and maintaining working relationships with Service Coordinators, practitioners, and community partners. The position also coordinates transition planning activities, facilitates intake and eligibility meetings, and Individualized Family Service Plan (IFSP) meetings. The role requires the ability to remain stationary up to 50% of the time and traverse various terrains to access visitation sites. Additional responsibilities include informing families and Service Coordinators of travel reimbursement processes, compiling and submitting reports, assisting in practitioner recruitment, participating in monitoring and continuous improvement activities, Child Find and public awareness activities, responding to inquiries, and attending relevant committees, conferences, meetings, and trainings. Any additional duties as assigned.

Requirements

• Valid driver’s license and reliable transportation.
• Adhere to the National Association for the Education of Young Children (NAEYC) Code of Ethics.
• Adhere to the Division of Early Childhood Council for Exceptional Children Code of Ethics.
• Maintain STARS career pathway enrollment.
• Maintain ISC credential.
• Able to travel extensively; requires the need for flexible scheduling, including occasional evenings and weekends.
• Business casual apparel.
• Mobility within the office including movement from floor to floor.
• Access information using a computer.
• Must be able to lift 25 pounds at times.

Nice To Haves

• This position may be eligible to work remotely up to two days per week following a successful 90-day review.

Responsibilities

• Contact assigned referrals within established timelines.

• Coordinate transition planning activities/conference with other early childhood programs.
• Facilitate the initial intake, eligibility meetings, and Individualized Family Service Plan (IFSP) meetings.
• Inform families and Service Coordinators of the travel reimbursement process.
• Compile and submit reports within established timelines.
• Assist in recruiting practitioners in identified areas of need.
• Participate in ongoing monitoring and continuous improvement activities.
• Actively participate in Child Find and public awareness activities.
• Respond to any inquiries within a timely manner.
• Participate in all required committees, conferences, meetings and trainings relevant to the program and/or agency.
• Any additional duties as assigned.
