About The Position

ALCOTT, A LEADING NONPROFIT AGENCY DEDICATED TO PROVIDING VITAL MENTAL HEALTH AND SUPPORTIVE HOUSING SERVICES TO LOW-INCOME ADULTS ACROSS LOS ANGELES COUNTY, IS SEEKING A COMPASSIONATE AND DEDICATED INTERIM HOUSING CASE MANAGER TO JOIN OUR INTERIM HOUSING TEAM. Requirements

  • Bachelor's degree in Social Work, Psychology, Mental Health, Human Services, or a related field with at least one (1) year of experience working with individuals experiencing homelessness; OR High school diploma or GED with at least two (2) years of relevant experience serving individuals experiencing homelessness.
  • Knowledge of trauma-informed care, harm reduction, crisis intervention, and housing-first principles.
  • Familiarity with public benefits programs, housing navigation, and community-based resources.
  • Strong communication, documentation, organizational, and advocacy skills.
  • Ability to effectively manage multiple clients and priorities in a fast-paced environment.
  • Proficiency using electronic documentation systems and Microsoft Office applications.
  • Valid California Driver's License, acceptable driving record, and ability to meet Alcott's insurance requirements if driving is an essential job function.
  • Successful completion of Live Scan and background screening.

Nice To Haves

  • Experience working with individuals living with serious mental illness, substance use disorders, or co-occurring conditions is highly preferred.

Responsibilities

  • Provide comprehensive case management services to individuals residing in Alcott's interim housing programs, helping clients overcome barriers to housing while connecting them with essential community resources and supportive services.
  • Welcome New Clients: Meet with new program participants within 24 hours of admission to complete intake, provide program orientation, review house guidelines, and help establish a welcoming environment.
  • Build Trust: Utilize trauma-informed care and harm reduction principles to foster meaningful client relationships and encourage engagement in services.
  • Ensure Compliance: Complete required intake documentation within established timelines and coordinate with the Program Manager when exceptions arise.
  • Coordinate Care: Collaborate with Residential Aides and medical providers to support room assignments, medication delivery, and client needs during admission.
  • Develop Individualized Plans: Complete assessments, develop personalized care plans, and support clients in achieving housing and personal recovery goals.
  • Provide Ongoing Support: Conduct weekly client meetings, monitor progress, address barriers, and document all services in CHAMP.
  • Connect Clients to Resources: Assist clients with obtaining identification, public benefits, medical care, transportation, employment resources, and permanent housing opportunities.
  • Advocate for Success: Coordinate appointments, communicate with service providers, and advocate for client needs throughout their housing journey.
  • Maintain Accurate Records: Ensure client files, assessments, consent forms, and case notes remain complete, accurate, and current.
  • Monitor Progress: Provide regular updates to the Program Manager regarding client progress, challenges, and service outcomes.
  • Collaborate Across Teams: Participate in interdisciplinary meetings, review Residential Aide shift reports, and coordinate care with internal and external partners.
  • Support Program Operations: Complete 5x5 assessments, maintain required documentation, and perform additional responsibilities as assigned.
