About The Position

The Interdisciplinary Team Conference Coordinator oversees and coordinates the inpatient rehabilitation team conference process to ensure compliance with CMS regulations, documentation requirements, and organizational standards. This role partners with physicians and interdisciplinary teams to monitor conference completion, identify compliance risks, support standardized workflows, and ensure accurate documentation of patient care planning activities.

Requirements

  • Associates degree or equivalent training required through, at least, five (5) years of progressive on-the-job experience.
  • Advanced understanding of anatomy & physiology, medical terminology, pharmacology, and record documentation requirements to appropriately apply IRF PPS criteria.
  • Familiarity with electronic health record (EHR) systems.
  • Strict adherence to HIPAA guidelines and patient privacy rules.
  • Quick learning style with the ability to ask discerning questions.
  • Sense of urgency and ability to quickly adapt to changes in priorities.
  • Knowledge of healthcare documentation practices and interdisciplinary care processes.
  • Familiarity with electronic health record (EHR) systems.
  • Strong organizational skills with the ability to manage multiple priorities and deadlines.
  • Effective verbal and written communication skills.
  • Proficiency with Microsoft Office applications, particularly Outlook, Word, and Excel.
  • Strong attention to detail and ability to identify documentation or process concerns.
  • Ability to work independently and collaboratively with interdisciplinary teams.
  • Excellent analysis and problem-solving skills.
  • Information-management skills and ability to interpret, communicate, and present data.

Nice To Haves

  • Experience as a RHIA, RHIT, COTA, PTA, or LPN is preferred.
  • Exposure to CMS regulations, compliance monitoring, or quality improvement activities is preferred.

Responsibilities

  • Coordinates interdisciplinary team conferences to ensure compliance with CMS, accreditation, and organizational requirements.
  • Monitors patient scheduling and conference completion to ensure required meetings occur within established timeframes.
  • Ensures required disciplines participate in conferences and physician concurrence is appropriately documented.
  • Provides real-time oversight of conference processes and identify potential documentation or compliance gaps.
  • Maintains tracking tools, reports, and dashboards to monitor team conference compliance and performance.
  • Audits conference documentation for completeness, accuracy, and regulatory compliance.
  • Develops and maintains standardized workflows, checklists, and discussion guides to support consistency and compliance.
  • Identifies and communicates changes in regulatory requirements and assist with implementation of process improvements.
  • Escalates compliance concerns and collaborate with stakeholders to resolve documentation or workflow issues.
  • Serves as a resource to clinical teams regarding team conference requirements, documentation standards, and compliance expectations.
  • Supports internal audits, accreditation activities, and regulatory readiness efforts.
  • Collaborates with clinical, HIM, revenue cycle, and operational teams to reduce compliance risk and support reimbursement integrity.
  • Demonstrate excellent customer service and standards of behaviors as well as encourages, coaches, and monitors the same in team members.
  • This individual should consistently promote teamwork and direct communication with co-workers and deal discretely and sensitively with confidential information.
  • Contribute by identifying problems and seeking solutions.
  • Promote patient/family satisfaction where possible; participates in departmental efforts to monitor and report customer service.
