About The Position

We are actively seeking a compassionate and skilled Intensive In-Home Qualified Professional to join our team full-time. This role is essential to our mission, providing therapy, crisis support, and service coordination for mental health clients in a variety of settings. As a Intensive in-Home Qualified Professional with Carolina Outreach, you will provide community-based/in-home individual and family therapy for children, youth, adolescents, adults and their families. Position Description: Serve as a member of the client’s clinical treatment team and create, develop, monitor, revise, and implement the client’s Person-Centered Plan. Participate in the development of crisis plans and transition plans when clients move in and out of services. Provide individual and family focused support to clients and their families/natural supports based on their needs, diagnoses and best evidence-based practices. Provide any written documentation as required. This will include treatment notes, goals and plans for the medical record (e.g., Service notes, person-centered plan etc.) Direct Services Work to implement therapeutic interventions, crisis interventions, behavioral interventions, psycho-educational activities and other best evidence-based practice therapies. Work with clients, families, natural supports and collateral agencies in reviewing, applying and achieving goals as indicated in the Person-Centered Plan. Requirements

  • At a minimum, a Bachelor's Degree in social work, or psychology and 2 years experience in the behavioral health field.
  • An employee must become NVCI certified; receive training in population served, and client rights/confidentiality.
  • Employees will be required to complete training in the evidenced based family therapy model selected by agency as well as crisis response training as responding to family crises is an expectation of the role.
  • A valid driver’s license and current automobile insurance is also required.
  • An employee must demonstrate having the core skills to deliver appropriate services prior to service provision and be able to initiate tasks and work independently as a member of the clinical team.
  • Employees must complete all agency and state-mandated trainings within required timeframes.

Responsibilities

  • Provide parent/guardian/natural support training that will aid persons interacting with the client in dealing more effectively with the client.
  • Assist in all service delivery as formally planned with the clinical team.
