We are looking for an Intensive Family Based Services Coordinator to join our Easterseals VT team! The individual in this key position will be responsible for writing and implementing plans of care, writing contact notes, formal monthly reports and discharge summaries, and maintaining frequent communications with Family Service Workers to ensure compliance with the family’s case plan. This role involves providing services to families within their home and community settings, assisting families in finding resources for preventative and supportive services (e.g., housing, transportation, substance abuse treatment, counseling, and nutrition). The coordinator will help caregivers recognize their children’s needs, make plans, and implement strategies to meet those needs, thereby growing their confidence and skills while ensuring a safe environment for their children. The position requires participation in meetings, conferences, and training events as assigned, providing transportation for clients when required, and testifying concerning services in court when subpoenaed. The role also ensures Family Assessments are completed in a timely manner as prescribed by IFBS policies, creates crisis plans, responds to crisis calls as needed, and provides in-home family support during times that work for the family. A key function is implementing short-term (13-week) intervention and support to prevent DCF involvement or out-of-home care, and/or to reunify children with their families, ultimately improving family functioning. The coordinator will also coordinate services with other providers, such as Schools, Early Intervention services, clinicians, and other relevant service providers.
