  • Cross-cultural experience and/or knowledge of the refugee/immigrant experience.
  • Highly self-motivated with strong follow-through and ability to work independently.
  • Strong mediation and problem-solving skills.
  • Strong organizational skills and ability to maintain detailed, accurate records.
  • Strong interpersonal and verbal communication skills.
  • Strong Microsoft Office skills required.
  • Ability to work a flexible schedule including occasional early mornings, evenings, and/or weekends.

Nice To Haves

  • Bachelor's degree or higher in social work, public health, or related field preferred.
  • Fluency in Dari, Russian, Arabic, French, or Spanish is highly desired.
  • Able to supervise interns is preferred.
  • Comfortable working in a team environment.
  • Direct-service experience with vulnerable populations preferred.

Responsibilities

  • Create individual client service plans with specific goals leading to client independence within the program period, managing a caseload of approximately 20 individuals from various funding sources.
  • Provide case management supportive services for individuals funded through Medicaid, including conducting necessary assessments and follow-up through IM+CANS.
  • Conduct home visits and assess ongoing needs to assist clients in meeting service plan goals.
  • Provide orientation on various topics and assess life skills in areas such as financial literacy, parenting, mental health, navigating healthcare systems, and digital literacy.
  • Identify specialist medical providers and coordinate client appointments to ensure timely medical care.
  • Assist clients in navigating health insurance requirements for accessing medical specialist care and educate them on working with health insurance to pay medical bills.
  • Provide transportation to medical and medical-related appointments, assisting clients in becoming independent in navigating appointments.
  • Assist clients in arranging transportation for follow-up appointments and establishing future transportation plans.
  • Make appropriate referrals to community agencies for services not provided internally.
  • Effectively exit clients from intensive service provision at the conclusion of the program period, including referrals to internal departments and/or outside entities.
  • Coordinate with outside agencies/organizations to meet client needs, advocating and mediating on behalf of clients.
  • Coordinate with primary care providers, specialist medical providers, and medical insurance providers to ensure appropriate and timely medical care.
  • Coordinate with other WRDA teams and departments to provide strong collaborative care.
  • Participate in weekly case planning meetings with direct service staff and program management staff.
  • Ensure monthly reporting information is submitted in a timely and accurate manner, meeting all Preferred Communities (PC) program requirements.
  • Attend PC monthly meetings and required training or HO annual conferences.
  • Complete program-required forms within specified time frames, ensuring accurate recording and filing.
  • Write detailed case notes for each task/client interaction, maintaining records in Ritten (for Medicaid) and Dynamics (for all else).
  • Retrieve participant signatures and complete all necessary program paperwork.
  • Write monthly stories and provide input for grant reports.
  • Research topics and gather information for group sessions, utilizing evidence-based practices and peer-reviewed materials.
  • Maintain accurate records of client files, case notes, new providers, use of interpretation, and reporting records.
  • Actively participate in department meetings, staff development opportunities, program trainings, community activities, and organizational events.
  • Travel regularly to clients' homes and aid in connecting them to other service providers and group sessions.
