About The Position

The Intensive Case Manager (ICM) will demonstrate a compassionate and caring nature, with an unbiased, non-judgmental, and person-centered approach. The role involves working with clients experiencing mental illness, substance use, and barriers to resources for health, wellbeing, vocation, and education. The ICM will build trust by involving clients in discussions about their goals and recovery process, grounded in regular assessment of client-centered needs. Toronto. The ICM will provide service in a dignified, culturally competent, and respectful manner, reflecting an integrated anti-racism/anti-oppression framework. The role involves maintaining a caseload within provincial guidelines, engaging in the intake process, providing resource information, community facilitation, advocacy, and completing referrals. Utilizing a psychosocial rehabilitation approach, the ICM supports clients in achieving goals that enhance their quality of life, liaises with formal and informal supports with client consent, assists in developing life skills, and helps clients maximize competencies for self-sufficiency. The ICM works from a strengths-based perspective within an empowerment framework and keeps the Program Manager informed of client developments. Adherence to health and safety rules and compliance with the Occupational Health and Safety Act are also required.

Requirements

  • University Degree in Social Work, Nursing, Occupational Therapy or related field
  • Addictions Certificate from a recognized college
  • Two years' work-related experience, particular with persons who experience problematic substance use and mental health challenges.
  • Extensive knowledge of the social service system and community resources.
  • Expertise providing counselling and case management to individuals who are experiencing or have experienced a complex history of substance use and mental health concerns.
  • Crisis intervention and risk assessment skills, with well-developed problem-solving abilities and interpersonal skills.
  • Demonstrated knowledge of crisis intervention de-escalation strategies and interventions and an ability to remain calm in crisis situations.
  • Demonstrated ability to recognize and respond to the emotional, psychological and physical needs of individuals reacting to a crisis situation.
  • CPR certification or willingness to be certified is required
  • Committed to anti-racist/anti-oppressive practice and to the mission and values of Good Shepherd
  • Demonstrated understanding of workplace health and safety practices and understanding of an employee's responsibility under current legislation.

Responsibilities

  • Maintains a case load within the provincial guidelines for Intensive Case Management.
  • Engages in the intake process including, liaison with Access Point, interviewing potential clients, utilizing appropriate assessment tools to determine the goals and needs of clients, client summary reports, client discharge/transfer reports, maintaining client files, client progress notes and completion of all other necessary paperwork and assignments.
  • Provides resource information, community facilitation, and advocacy for clients as needed. Completes referrals as required.
  • Utilizing a psychosocial rehabilitation approach to care, supports clients in achieving goals that enhances the client's enjoyment and quality of life.
  • With client consent, liaises with clients' formal and informal supports.
  • Assists to enhance and develop necessary life skills as needed.
  • Assists the client to maximize their competencies, in order to become more self-sufficient
  • Works from a strengths-based perspective within an empowerment framework.
  • Keeps the Program Manager informed of developments and changes concerning clients on an as needed basis.
  • Observe all health and safety rules and work in compliance with the provisions of the occupational Health and Safety Act and its regulations
