About The Position

Mental Health America of Los Angeles (MHALA) is a non-profit mental health agency dedicated to helping individuals with mental health needs achieve meaningful, healthy lives. This role specifically focuses on providing Intensive Case Management Services (ICMS) to clients eligible for permanent housing through various programs, and also serves as a support for housing case management at a Project Base site and for residents at Beacon Pointe Project Base community, fostering community integration through group activities and events.

Requirements

  • High school diploma or GED with a minimum of 1 year of experience providing psychosocial rehabilitation-related services to a disabled population.
  • Proficiency in computer applications such as Microsoft Word, Excel, and Outlook.
  • Ability to learn and utilize Homeless Management Information Systems (HMIS) and Electronic Health Records Systems (EHR).
  • Experience working with clients with mental illness, chronic health issues, and substance use disorders.
  • Expertise in chronic homelessness, outreach and engagement strategies, intensive case management services, best practice models, mental health and substance use disorder services, crisis intervention, suicide assessment and prevention, affordable housing and public benefits applications, housing and landlord/tenant rights, and eviction prevention.
  • Ability to maintain a caseload ratio of one (1) full-time equivalent intensive case manager to 20 to 25 clients with a VI-SPDAT of 1 or 2.
  • Flexible work schedule to include some early mornings, nights, weekends, and holidays.
  • Valid class C driver's license.
  • Access to a vehicle for transportation.
  • Proof of valid automobile insurance.
  • Ability to be insured by MHALA’s liability insurance.
  • TB clearance and re-testing every year.
  • Employment verification directly from three (3) past employers within the most recent 7-year period.

Responsibilities

  • Assist individuals at every stage of the housing stabilization process, offering flexible services to meet client needs.
  • Conduct outreach and engagement, process referrals, build rapport with clients, conduct face-to-face screenings, obtain consents, and link clients to other necessary providers and services.
  • Perform intake and enrollment activities, assist with program eligibility documentation, complete project intake forms, and enroll clients into ICMS.
  • Assist with temporary housing referrals until permanent housing is secured.
  • Ensure all documentation is prepared and entered into appropriate systems monthly.
  • Support clients through the process of moving into permanent housing, including document readiness, application assistance, coordinating move-ins, and orienting new tenants.
  • Assist clients in accessing services and addressing immediate needs, conduct comprehensive psychosocial assessments, and develop individualized case management service plans in collaboration with clients.
  • Help clients obtain income, establish benefits, find employment or educational opportunities, and assist with basic needs, life skills, community participation, independent living skills, budgeting, legal issues, and transportation.
  • Complete all paperwork in compliance with applicable laws, professional licensure requirements, MHALA, CHAMP, LACDMH County Contracted Provider standards, and funding source requirements, documenting in EHR within 24 hours.
  • Develop, implement, and explain a comprehensive orientation for new residents at Beacon Pointe, covering behavior, lease conditions, and building rules.
  • Manage day-to-day resident and building logistics at Beacon Pointe.
  • Be present and available on-site for residents and building maintenance at Beacon Pointe.
  • Create and maintain annual housing documents and updates for Beacon Pointe residents.
  • Maintain a separate electronic chart for each participant at Beacon Pointe, including service plans, mental health provider contact information, documentation, and progress notes within 24 hours.
  • Be knowledgeable of community services and supports and coordinate linkages for tenants at Beacon Pointe.
  • Conduct ongoing assessments and evaluations to monitor progress and provide interventions as needed at Beacon Pointe, coordinating with the Personal Service Coordinator or other mental health providers.
  • Create and update tenant service plans annually for all units at Beacon Pointe.
  • Act as a liaison among property management, tenants, mental health providers, housing authorities, and other service providers to facilitate communication and prevent issues that could jeopardize residency.
  • Provide life skills training, including community integration, housekeeping, and neighborly conduct.
  • Enter notes into the Electronic Health Record within 24 business hours and maintain all records for program compliance.
