About The Position

The Intensive Case Management Worker at Harbour House HOPE is responsible for providing comprehensive support and life skills training to participants, assisting them in achieving independent living and transitioning to stable housing. This role involves creating individualized service plans, coordinating with various professionals, and advocating on behalf of participants. The position also includes program support, such as developing housing stability plans and maintaining relationships with housing authorities, as well as administrative duties like maintaining detailed records and ensuring participant files are up-to-date.

Requirements

Provide scheduled assessments (SPDAT & other assessments as required)
Creating Individualized Service Plan (ISP) with participants.
Assist Participants as needed in developing and maintaining a positive working relationship with professionals in support of ongoing physical, spiritual, emotional, and mental wellness.
Assist and encourage Participants in the pursuit of leisure activities, exercise programs, and community socialization opportunities; develop activities for individual or group participation.
Coordinate and participate in group and recreation activities as needed.
Support Participants in management of self, i.e., development and utilization of personal coping strategies, self confidence & community rehabilitation.
Assist Participants in realizing personal daily living goals, both short and long term, to facilitate progression towards independent living and towards the transition to more permanent housing.
Provide Participants with guidance, information, and referrals pertaining to personal finance and legal issues, assist participants in navigating and understating systems processes.
Assist in navigating referral pathways and overcoming barriers to service whenever appropriate.
Support Participants in becoming active members of the community at large.
Act as a positive role model for Participants; lead by example.
Develop housing support plans with an emphasis to housing stability factors.
Develop and maintain relationships with local housing authorities, housing management bodies and private landlords.
Encourage Participants to actively participate in the treatment plan(s) and assist them in understanding and taking ownership of their individual and community treatment plans
Assist Participants in scheduling appointments and attend when necessary; offer emotional support and advocate on behalf of the Participant and the program when necessary.
Advocate on the Participant's behalf whenever necessary.
Perform regular assessments.
Monitor Participant interactions and encourage social inclusion; coordinate conflict resolution and manage behaviours as needed.
Maintain personal and professional boundaries with Participants.
Maintain supportive communication and provide on-going support as needed.
Actively participate in professional development opportunities; attend as many trainings and in-services as possible.
Assist with the orientation, training, mentoring, and ongoing support of fellow team members.
Attend all mandatory trainings (Conditions of Employment).
Attend other meetings as required or assigned.
Actively work to be a positive and supportive team member.
On call duties as directed by the Operation Managers.
Comply with the YWCA Lethbridge & District policies and procedures.
Maintain regular communication with the Participants’ primary support network (Counsellor, Therapist, Psychiatrist, Physician) on issues concerning overall wellness. Document all third-party communications.
Maintain detailed records according to the YWCA Lethbridge & District legal documentation guidelines.
Maintain Participant files and records as per YWCA Lethbridge & District guidelines.
Ensure Participant files are complete and up to date, including timely completion of case notes, medication information, progress reports, goals, and any other relevant details.
Support with the collection of Service Fees
Complete all reports and summaries, including monthly statistics, and discharge summaries, in a timely manner according to YWCA Lethbridge & District documentation guidelines.
Assign duties as needed to the Support Worker(s); the Case Management worker is not to perform the duties of a supervisor, but can provide direction with regards to Participant care, particularly in relation to the Service Plan and Participant Goals.
