JRI's Community Service Agencies are committed to delivering comprehensive wraparound services for youth under 21 who are diagnosed with a serious emotional disturbance. These services cater to those who need multiple forms of support or are engaged with various child-serving systems, including child welfare, special education, or juvenile justice. In this role, you will have the opportunity to support clients to achieve their goals by leading Care Planning Teams, coordinating various services and supports for MassHealth-enrolled children and youth under the age of 21 who have serious emotional or behavioral health needs. You will serve as a liaison between individuals, partnering agencies, and supportive programs or services to facilitate the care of youth. You will identify and help families access available services and natural supports for both youth and families, while also monitoring their effectiveness. You will provide trauma-informed, wraparound services that prioritize the needs of the youth. You will assist parents and caregivers in developing effective advocacy skills.
