SCOPE OF ROLE Under the supervision of the Ryan White Program Manager, the Intensive Behavioral Health Medical Case Manager provides a range of client-centered services that link clients with medical, psychosocial, and other needed services. The coordination and follow-up of medical and behavioral health treatments is a key component of intensive behavioral health medical case management. These services ensure timely, coordinated access to medically appropriate levels of health and support services and continuity of care through ongoing assessment of the client’s and client’s key family members’ needs and personal support systems. Client and family encounters may include face-to-face and phone contact as well as other forms of communication. Visits will be office based and/or home and/or community based per the individualized care plan. The goal of the Intensive Behavioral Health Medical Case Manager is to focus more intensively on behavioral health issues of Ryan White clients in order to address barriers to consistent medical care and medication management which may lead to greater likelihood of viral load suppression.

Requirements

  • Ohio license for LSW or LISW
  • Must have formal training as well as practical experience in medical case management models and possess core and specialty competencies and skills working with the HIV/AIDS client population, as well as those diagnosed with mental health and substance use disorders.
  • Must have knowledge, awareness, sensitivity, comfort, and competence regarding implicit bias, cultural competence and humility, health disparities, LGBTQ+ communities, people of color, substance use disorders, sexual health, and trauma-informed care.
  • Reliable transportation and willingness to travel throughout Cuyahoga county and occasional meetings and trainings outside of Cuyahoga county.
  • Valid Ohio driver’s license and good driving record.
  • Able to work in varied environments
  • Regularly required to stand, sit, walk, bend, reach with hands and arms,; talk and listen on a regular basis
  • Excellent communication skills both oral and written.
  • Experience working in a behavioral health setting providing consultation, intervention, and referral for psychological/psychiatric problems
  • Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral health conditions.
  • Ability to design and implement clinical pathways and protocols for treatment of selected chronic conditions.

Responsibilities

  • Provide comprehensive, strengths-based, client-centered assessment of behavioral health needs.
  • Develop a comprehensive, individualized care plan specific to behavioral health needs to improve medical outcomes.
  • Coordinate medical and behavioral health services required to implement the plan.
  • Evaluate crisis situations and applies appropriate interventions.
  • Assist in identifying “at-risk” clients and develops plans to prevent psychological and/or physical health barriers.
  • Provide education to clients, families, and staff about care, prevention, and treatment enhancement techniques.
  • Continuously monitor clients to assess the efficacy of the plan.
  • Periodically re-evaluates and adapts clients’ individualized plans.
  • Provide a range of client-centered services that link clients with health care, psychosocial, and other services, including benefits/entitlement counseling and referral activities, assisting the client to access other public and private programs for which the client may be eligible (.g., Medicaid, Medicare Part D, State Pharmacy Assistance Programs, and other State or local health care and supportive services).
  • Perform ongoing assessment of high-needs or at-risk client’s and client’s key family members’ needs and personal support systems.
  • Perform treatment adherence counseling to ensure readiness for, and adherence to, complex HIV treatments.
  • Maintain client charts and ensures the completion/documentation of required elements for compliance with contractual and Ryan White programmatic requirements, including required case management activities, services, and activities, the type of contact, and the duration and frequency of the encounter.
  • Perform client-specific advocacy and/or review of utilization of services.
  • Maintain clear communication with the entire treatment team, including primary medical case manager, medical team, behavioral health team, referral sources and other designated liaison persons.
  • Monitor and enforce client’s rights and confidentiality, and ensures high ethical standards.
  • Conduct and participate in all Quality Improvement and Utilization Review activities.
  • Personally maintains a high standard of professional and ethical standards.
  • Meet or exceed productivity and direct service expectations.
  • Comply with all agency policies and procedures.
  • Maintain all aspects of HIPAA, promoting patient confidentiality and right to privacy.
  • May be required to perform telehealth services as determined by Signature Health.
  • Adherence and completion of compliance training provided by Signature Health.
  • Responsible for and completes all productivity/documentation requirements.
  • Participate in all assigned staff meetings and staff development programs.
  • Demonstrate appropriate attendance and punctuality in adherence with Signature Health policies.
  • Other duties as assigned.
