Conduct thorough evaluations of patients to assess mental health needs and substance use patterns to determine appropriate plan of care and treatment needs. Develop, implement, and modify personalized treatment plans tailored to the specific needs of each patient. Monitor for changes in status or goal completion. Provide counseling, using a variety of therapeutic modalities, to patients with co-occurring substance use and mental health diagnoses in group/individual sessions to assist the patient with successful transition a community-based treatment model. Provide psychoeducational to patients and their families about co-occurring disorders, considerations for treatment, strategies for symptom management, and available community-based resources. Collaborate with a multidisciplinary team of psychiatrists, psychologists, case managers, and nurses to plan and coordinate treatment to best meet the patient’s needs. Maintain accurate and timely documentation of all assessments and interventions in compliance with agency and regulatory standards. Engage in regular professional development activities and continuous education opportunities in accordance with relevant licensure requirements. Other duties as assigned.
