About The Position

Reporting to the Resource Coordinator, the ISW (Integrated Support Worker) will have knowledge and training in personal care skills and be a certified Health Care Aide (HCA). Preferably, the ISW will also have additional training in working with individuals experiencing mental health or behavioral challenges but additional training may be obtained after start date. The ISW is responsible for client care involving Health Care Aid duties and supportive care duties as directed by the Case Coordinator/Resource Coordinator. This includes partnering with clients to improve their wellness in community, social functioning, self-care, communication, self-help, and coping. ISWs are located in several housing projects that operate in a 24/7 team model, as well as in the community at large.

Requirements

  • WRHA recognized HCA certificate required.
  • Grade XII minimum or GED equivalent.
  • Valid driver’s license and vehicle required.
  • May be required to provide a Personal Driver’s Abstract.
  • Knowledge of Personal Health Information Act (PHIA), Workplace Hazardous Material Information System (WHMIS) and Principles of Routine Practices (Universal Precautions).
  • Knowledge of Regional and Program policies and procedures.
  • Knowledge of Mental Health First Aid.
  • Knowledge of SafeTALK.
  • Knowledge of WRHA Violent Prevention and Protection Modules and/or Non Violent Crisis Intervention.
  • Knowledge of Roles and responsibilities of members of the healthcare team.
  • General skills and ability to perform all tasks and responsibilities relevant to the duties of the ISW position.
  • Ability to provide safe, effective personal care in a manner that considers each individual’s capabilities and capacity.
  • Ability to provide safe, effective personal care in a manner that considers each individual’s need for comfort, privacy, and respect is essential.
  • Understanding of client empowerment and issues faced by individuals experiencing challenges with mental and/or physical health issues.
  • Ability to communicate effectively with clients, family, and coworkers is essential.
  • Ability to assess and teach basic life skills.
  • Ability to be flexible to meet client care needs.
  • Ability to set up and maintain boundaries with clients.
  • Ability to perform as a team member and seek assistance as needed.
  • Effective English verbal and written communication skills.
  • Current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check.

Nice To Haves

  • Additional training in working with individuals experiencing mental health or behavioral challenges.
  • Two (2) years’ experience as an HCA (Home Care Attendant /Health Care Aide) preferred.
  • Two (2) years’ experience working in Mental Health preferred (may include experience as an HCA working with clients with complex needs or behaviors).
  • Competent in an Aboriginal language and/or knowledge in Aboriginal customs, traditions and values.
