The Integrated Care Specialist will explain available services and focus on client engagement to access services. This role involves completing health screenings, bio-metrics, and facilitating outside referrals as directed by the care team. The specialist will participate in team meetings to develop a plan of care to address client needs and assist clients in navigating the healthcare environment to ensure access to appropriate care. They will advocate on the client's behalf with healthcare providers and managed care organizations to ensure timely care. Additionally, the role assists with coordination and access to services that address social determinants of healthcare outcomes, including transportation, housing, and other community resources like public assistance and financial benefits. The specialist will develop a knowledge base of other healthcare providers in the service area and collaborate as needed, document tracking for appointments, medication, and other healthcare needs, and provide education and support for wellness activities. They will also provide caring contacts to clients at high risk for suicide to ensure continuity of care and collaborate with clients, family/significant others to develop support networks.
