About The Position
DaVita Integrated Kidney Care (DaVita IKC) is the integrated care division of DaVita Inc., working on DaVita’s vision to provide integrated care to all ESRD patients, who are some of the most medically complex and vulnerable patient populations in the US. We provide services to help improve the lives of our patients by working with them to prevent complications, reduce the number of avoidable hospitalizations and improve overall health.
